Day one post craniotomy for right subdural collection
Patient Data
Right parietal craniotomy with underlying mixed density right cerebral convexity subdural collection.
Moderate volume pneumocaphalus deep to the craniotomy.
Leftward midline shift.
Subdural hematoma of the right tentorium cerebelli.
Two new areas of parenchymal hemorrhage of the right cerebellar hemisphere and a smaller petechial hemorrhage of the left cerebellar hemisphere in keeping with remote cerebellar hemorrhages.
Case Discussion
Remote cerebellar hemorrhage is a rare complication of supratentorial surgery. The exact etiology is unknown but one postulated cause is CSF over drainage and descent of the cerebellum. The cerebellar hemorrhage can be on the ipsilateral, contralateral side (to the surgery), or both, as in this case.
