Pisiform fracture is undisplaced. Tiny calcific foci at the tip of the ulnar styloid may represent acute avulsion fractures. Associated soft tissue swelling centered on the ulnar aspect of the wrist.

There is a minimally displaced fracture of pisiform. Elsewhere no other acute fractures and no joint malalignment identified. A well corticated bony fragment at the dorsal aspect of the third carpometacarpal joint compatible with a carpal boss.
Case Discussion
Fracture of the pisiform is one of the less common carpal fractures. The ulnar-sided wrist swelling is a helpful clue that there may be an injury to side of the wrist.
