Chronic cyclic abdominopelvic pain and concern for acute bowel obstruction
Patient Data







Complete duplication of uterine horns, cervix, and vagina. The right hemivagina and uterus appear distended by fluid (hematometrocolpos), while the left appears collapsed. Terminal ileum irregular wall thickening, focal distention and appearance of adhesions are suggestive of partial bowel obstruction. Abdominopelvic free fluid is seen. Right kidney is absent (on the same side of the vaginal obstruction).
Case Discussion
Herlyn-Werner-Wunderlich syndrome, also known as obstructed hemivagina and ipsilateral renal anomaly (OHVIRA), is a rare condition characterized by Mullerian duct anomalies associated with mesonephric duct anomalies.
In this case, histopathologic specimen from the terminal ileum and appendix showed findings of involvement by endometriosis.
