There is a relatively well defined cystic lesion, midline posterior to urinary bladder in prostatic region, appears hyperintense in T2W images, shows no enhancement in T1W post contrast scan, extending superiorly to middle of seminal vesicles. It demonstrates a pear-like shape, without clear communication with urethra.

Also noted little prostatic tissue.

Case Discussion

Midline cystic lesion in prostatic region, extending superiorly above the prostate,  with no communication with urethra, suggestive of mullerian duct cyst.
