Anterior shoulder dislocation during self-defense training. Since the initial event, he complains of diffuse shoulder pain, weakness, and instability.

A fat-suppressed axial T1-weighted MR arthrography demonstrates an avulsed anteroinferior labrum (3-6 o'clock) with scapular periosteum intact but stripped medially.

Case Discussion

The Perthes lesion is a variation of the Bankart lesion, where the scapula periosteum is lifted and stripped medially with the detached anterior labrum (3-6 o'clock). The Perthes lesion may be demonstrated by fluid extending beneath the labrum and filling the space between the elevated periosteum anteriorly and the scapula posteriorly.
