Trauma due to motor vehicle accident; altered mental status (GCS 6).
Patient Data

Evidence of bilateral putamen hypodensity.
Also present, a diffuse subcortical white matter hypodensity.
Note that the patient is intubated.
Case Discussion
Methanol poising exhibits classically as bilateral putamen hypodensity, with associated features of subcortical white matter hypodensity.
This patient had a trauma due to a motor vehicle accident after returning from a party in a region where methanol intoxication is sporadically found. At the entrance to the emergency department, she had a very altered mental status, however, it was noted that there was an alcohol smell. This leads to a possible diagnosis of methanol poisoning.
The authority was notified, and an investigation was conducted. It later found out a dangerous practice of a local distiller of putting methanol into its local whiskey to make it "stronger".
