The anterior suprapatellar fat pad is edematous and enlarged. The posterior convex margin of the suprapatellar fat pad exerts mass effect over the suprapatellar recess.

Mild joint effusion is present.

Focal grade II chondral injury at the lateral patellar facet and mild prepatellar soft tissue edema are noted.

Case Discussion

Findings are consistent with anterior suprapatellar (or quadriceps) fat pad impingement syndrome.

However, bear in mind that edema and mass effect of the suprapatellar fat pad are common findings at routine MRI examinations of the knee. These findings should be interpreted based on additional imaging findings such as quadriceps femoris tendinitis, joint effusion, and appropriate clinical grounds 1,2.
