Chronic appearing dens fracture with posterior displacement of the cranial fragment. The space between the anterior arch of C1 and the dens (alantodental interval) is fused/mineralized, but otherwise appears intact. The remaining portions of the craniocervical junction are within normal limits.

There is grade 1 anterolisthesis of C4 on C5. The vertebral body heights are maintained.

This is a case of a chronic dens fracture.

Features in this case that aid in the diagnosis of a chronic, rather than an acute, fracture include the sclerotic margins of the fracture, as well as absence of soft tissue findings (edema or hemorrhage).
