Fahr syndrome following total thyroidectomy

Diagnosis almost certain

Presentation

Had total thyroidectomy with iratherapy ten years ago for papillary thyroid carcinoma.

Patient Data

Age: 30 years
Gender: Male

Bilateral and relatively symmetrical calcification, involving the dentate nuclei, cerebellum, globi pallidi, putamina, caudate nuclei, and subcortical white matter mainly at the frontotemporal regions.

Case Discussion

Blood chemistry revealed a low level of serum calcium and parathyroid hormone (PTH) and a high serum phosphate level, indicating hypoparathyroidism.

The clinical history correlated to the CT features and the result of blood chemistry is most consistent with Fahr syndrome due most likely to hypoparathyroidism following total thyroidectomy.
