Epigastric abdominal pain and tenderness with fever.

Evidence of an irregular lobulated thick-walled cystic structure with fluid-fluid levelling in the left liver lobe that is surrounded by increased vascularisation at colour Doppler in keeping with hepatic abscess.

Case Discussion

The diagnosis was proved by fluid aspiration.

Liver abscesses are usually ill-defined and variable in shape, however, in our case, the patient had maximum tenderness at the site of the abovementioned cystic structure/abscess, and was indicated as well by absence of central perfusion but increased peripheral vascularisation at Doppler.
