There is a high-attenuation MCA branch viewed on right Sylvian fissure. A small area of hypoattenuation is seen on adjacent parenchyma (ischemia).
Case Discussion
The dot sign represents a thromboembolus within a segmental branch of the MCA located within the Sylvian fissure. It's an early marker of thromboembolic occlusion.
