The left hippocampus appears relatively slightly smaller than the right and shows increased T2/FLAIR signal intensity. No significant temporal horn dilatation.

Features are highly suggestive of left-sided Mesial temporal sclerosis.

Case Discussion

Mesial temporal sclerosis is the most common association with intractable temporal lobe epilepsy It is initially treated with anti-epileptic agents, but in resistive cases surgery maybe needed.
