Presentation
Abdominal pain and progressive distention. Absent abdominal sounds on auscultation. No signs of toxicity and fever. No history of trauma or operation.
Patient Data

There is severe gaseous dilatation of the distal transverse, descending and sigmoid colon up to 13 cm without mechanical obstruction and frank mural abnormality. No pneumoperitoneum and no abdominopelvic free fluid are noted.
Case Discussion
Features are consistent with nontoxic megacolon, which refers to severe colonic dilatation without mural abnormality. This contrasts with toxic megacolon, an acute complication accompanied by mural abnormalities such as thickening, loss of haustral folds, pneumatosis or free gas.
