Well-corticated bony fragment adjacent to the medial malleolus represents os subtibiale.

Case Discussion

Os subtibiale is a rare accessory ossicle and represents a normal anatomical variation this is found incidentally on ankle imaging. It is usually asymptomatic and as with all accessory ossicles, it must not be confused for a fracture especially in the trauma setting.
