Presentation
Persistent non-resolving epigastric pain within the first week of sleeve gastrectomy.
Patient Data

Status post gastric sleeve operation showing positive contrast leakage surrounding the residual gastric pouch forming multiple small collections with internal loculated air and air-fluid levels seen within.
Ectopic pelvic right kidney, showing normal size and enhancement, no backpressure changes. Good opacification of mesenteric vessels with no abnormal filling defect or narrowing. Minimal left-sided pleural effusion with basal pleural reaction noted.
Case Discussion
Sleeve gastrectomy is a bariatric surgical procedure involving resection of the greater curvature of the fundus and body of the stomach. Leak after sleeve gastrectomy often occurs within the first few days postoperatively due to failure of staple line complete closure.
