Right basal airspace opacification with loss of the right heart border. Appearances are of airspace opacification in the right middle lobe and in the clinical context, this is infection.

Case Discussion

While airspace opacification is non-specific and could represent infection, fluid, hemorrhage or cells, in a child or adolescent, infection is almost always the cause, especially when there is an appropriate clinical history.

In children, there is often a degree of collapse in concert with airspace opacification.
