Medial knee pain, especially on weight-bearing. Partial medial meniscectomy had been performed 10 years earlier.

Cortical irregularity, flattening and articular cartilage erosions of the articular surface of the medial femoral condyle. This is associated with diffuse bone marrow edema of the medial condyle.
Partial medial meniscectomy of the posterior horn is noted.
Case Discussion
Features are suggestive of subchondral insufficiency fracture of the knee (previously known as spontaneous osteonecrosis of the knee), affecting the medial femoral condyle.
