An intra-spinal cystic lesion opposite lower sacral vertebrae compressing the thecal sac and nerve roots. It elicits CSF intensity low signal at T1 and high signal at T2 and STIR suggesting Tarlov cyst.

Case Discussion

A Tarlov cyst, also known as a perineural cyst, is a fluid-filled sac that forms within the nerve root sheath at the sacral region of the spine. These cysts are usually filled with cerebrospinal fluid (CSF) and are most commonly found at the S1-S4 nerve roots.

The radiological data of our case high suggest Tarlov cyst.
