Presentation
Progressive right calf swelling after a recent tennis match. The patient is on prophylactic warfarin therapy.
Patient Data

There is a myofascial injury with fluid and a hematoma, deep to the right medial gastrocnemius muscle and superficial to the right soleus muscle. A tendon strain of the medial gastrocnemius muscle is present.

The myofascial tear and hematoma between the medial gastrocnemius and soleus muscles are demonstrated in these video clips.
Case Discussion
Findings are typical of a tennis leg with a sizable myofascial tear and hematoma in a patient on prophylactic warfarin therapy. A complete complementary duplex doppler study revealed normal and patent deep and superficial veins.
