Testicular seminoma

Case contributed by Dr Bahman Rasuli

Presentation

Left testicular large mass sensation.

Patient Data

Age: 30 years
Gender: Male
MRI

The left testicle is enlarged and infiltrated by a large tumoral mass.

(right testicle: 29 x 32 x 46 mm = 21 ml.  Left testicle : 44 x 50 x 69 mm = 75 ml)

The tumor is heterogeneous but mainly of intermediate signal intensity on T2-weighted images. This is lower signal intensity than normal testicular parenchyma (as seen on the right).

After gadolinium administration, there is heterogeneous enhancement of the tumor associated with multiple internal enhancing septa.

Case Discussion

The patient underwent left-sided orchiectomy.

Microscopic description: "Specimen consists of a large testis well-encapsulated tumor. On cutting it is tumoral but the capsule strips off easily without distortion. The cut section is finely fibrillary. the spermatic cord is 9 cm long and 3cm in diameter showing on cutting increase in adipose tissue."

Final diagnosis: Seminoma
