Presentation

The patient presents with chronic pain in the medial aspect of the left leg below the knee joint.

There is evidence of bone marrow edema in the medial aspect of the left tibial proximal metadiaphyseal region with overlying cortical thickening and adjacent soft tissue edema signal. No discrete MRI-detectable lesion is identified, and no abnormal collection is noted. The remaining bones appear normal. There is no aggressive periosteal reaction, and no abnormalities are observed in the muscles.

Case Discussion

The presence of bone marrow edema in the medial aspect of the left tibial proximal metadiaphyseal region, without a discrete MRI-detectable mass/lesion, is highly suggestive of stress fracture. However, a CT scan can be considered to rule out the possibility of a small osteoid osteoma.
