Impending testicular infarction is a rare complication of epididymo-orchitis which can be diagnosed by absence or reversal of diastolic flow in intra-testicular arteries 1,2.
Clinical presentation
Patients may present with worsening of symptoms of epididymo-orchitis despite proper treatment.
Pathology
The mechanism of infarction following infection is not well understood. Many theories exist including inflammatory infiltrates and exotoxins causing end artery thrombosis 1.
Radiographic features
Ultrasound
absence or reversal of diastolic flow in intra-testicular arteries 1,2
surrounding edema including spermatic cord and scrotal wall is usually present
