Herniation of a large collection of small bowel loops (ileum) through a cecal mesenteric defect, behind and lateral to the cecum, and into the right paracolic gutter.

The herniated bowel is swollen and the mesentery is edematous with free intraperitoneal fluid = ischemic (at least venous).
