Large dilated loop of bowel centrally within the abdomen. The stomach is in normal position and there is no dilatation of the large bowel in general. Appearance therefore favors cecal volvulus.

There is massive distension of the cecum which reaches up to 14.5 cm in diameter. The cecal pole is rotated supero-anteriorly with a twisting motion nicely demonstrated in the right flank.
