Large vein of Galen aneurysm flow void with significant local mass effect: compression of the aqueduct and 3rd ventricle anteriorly with secondary hydrocephalus. The inferior sagittal sinus is draining into the aneurysm directly and straight sinus draining out (as seen on T1W sagittal imaging)
