Patient presented with unrelated complaint of abdominal pain and icterus. There was no respiratory distress.

Over-inflated left lung shows herniation across midline with rightward mediastinal shift and compression of right lung.

CT scan shows hyperinflation of left upper lobe and lingula with herniation across midline with contralateral tracheo-mediastinal shift and partial compression atelectasis of rest left lower lobe and right lung.
