Abdominal pain and distension.

Age: 60 years
Gender: Male

Opacified linear tract at posterior wall of gastric antrum connected to the lesser sac that is filled with fluid and gas contents, suggestive of perforated peptic ulcer.

Case Discussion

Posterior peptic ulcer perforation is a rare surgical emergency that carries high mortality rate due to late and insidious non-specific presentation and can be missed in laparotomy. Delayed presentation also can lead to peritonitis and multi-organ failure. Surgical treatment is the preferred management.
