Right loin pain with fever. Urine cultures were negative. A right renal mass was suspected on initial ultrasound (not shown).

Bilateral malrotated kidneys, in which the hila face anteriorly. This is seen in both the portal venous and delayed phases.
This study demonstrated renal malrotation, which is a normal variant. Poor drainage of the collecting system because of the position of the renal pelvis, can predispose the kidney to infection (due to urinary stasis) and stone formation.
