Bregmatic epidermoid cyst

Diagnosis probable

Presentation

Palpable anterior fontanelle soft and fluctuant mass since birth that was more prominent on crying.

Patient Data

Age: 6 months
Gender: Female

Axial CT-scan soft tissue and bone window images demonstrating the patent anterior fontanelle with an adjacent cystic lesion that looks CSF-density and certainly far denser than subcutaneous fat. On bone window, there appears to be a thin rim of subcutaneous fat seen between the cyst and the scalp.

Case Discussion

CT scan images of the skull confirm the extracranial position of the cyst which was not apparently communicating with the cranial cavity or superior sagittal sinus and was overlying the patent anterior fontanelle. An epidermoid cyst is felt most likely.
