Diagnosis almost certain

Patient Data

Age: 55 years
Gender: Male

Marked emphysema with upper lobe predominance. Minimal reticulation and ground glass changes in the lower lobes, more conspicuous on the right. In the right base there is subtle traction bronchiolectasis. The immediate subpleural regions in the dorsal lower lobes are spared and this is a typical feature of NSIP.
