The pons is diffusely enlarged encasing the basilar artery and exhibits altered signal being low in T1, slightly bright in FLAIR and very bright in T2 WIs. No diffusion restriction on DWI differentiating this lesion from acute pontine infarction.

Case Discussion

Features consistent of diffuse pontine glioma (histopathologically proved). Although information on the molecular characteristics of this tumor is not available, it is likely that it represents a diffuse midline glioma H3 K27M–mutant.
