Ectopic thyroid - anterior mediastinum

Diagnosis almost certain

Presentation

Shortness of breath.

Patient Data

Age: 70 years
Gender: Male

Hyperdense anterior mediastinal mass lying immediately below and of similar density to the left thyroid inferior pole. Further high-density but non-enlarged mediastinal nodes. Thyroid is not enlarged,

Peripheral reticular opacities both anteriorly and posteriorly without a clear apicobasal gradient. Mild bronchiolectasis in the bases. No honeycombing.

Technetium 99m pertechnetate

Nuclear medicine

Thyroid uptake study demonstrates focal tracer accumulation identified inferior to the thyroid gland at the level of the manubrium correlating with anterior mediastinum soft tissue nodule identified on the prior CT.

Case Discussion

The mediastinum is the least frequent location of ectopic thyroid tissue.
