The patient had unilateral proptosis

Left sphenoidal wing intraosseous expansile lesion display low signal in all pulse sequence with underlying meningeal enhancement in post contrast study and associated ipsilateral proptosis.

Right caudate nucleus old lacunar infarction.

Diagnosis: Meningioma en plaque (MRI features)

It is important to note that this case illustrates an intradural meningioma with intraosseous bone extension and not a primary extradural meningioma.
