Large well-defined retrorectal ovoid cystic mass of complex signal intensity containing serpiginous structures of low signal on T2 with surrounding fluid collection. No enhancement is seen on postcontrast sequences. No pelvic lymphadenopathy or free fluid in the peritoneal cavity.

Case Discussion

The clinical history and the MRI features are most consistent with a gossypiboma that was confirmed at surgery (surgical gauze).
