A well-defined, homogeneous hypoattenuating (HU=-84), infratentorial lesion in the region of the left paravermian location. No significant mass effect.

An extra-axial, non-enhancing T1 and T2-hyperintense signal lesion displaying signal loss in fat-saturated sequences, located adjacent to and above the left side vermis, expanding the paravermian cistern. The lesion exhibits blooming on the T2* GRE sequence.
Case Discussion
The features suggest a paravermian lipoma. The differential diagnosis could include a dermoid cyst, which is typically midline and located in the pineal region.
