Intracranial metastases

Presentation

New headaches and left hemiparesis in a patient being treated for non-small cell lung cancer.

Patient Data

Age: 45 years
Gender: Female
mri

Multiple nodules and masses are observed in the supratentorial region, with the largest lesion located in the right occipital lobe, measuring approximately 34 x 29 x 30 mm. The lesion is surrounded by perilesional edema and demonstrates heterogeneous signal intensity -predominantly hypointense on T1W, hyperintense on T2W and FLAIR, with mild diffusion restriction. Scattered hypointensities are noted on the GRE. Post-contrast, the lesion shows intense heterogeneous enhancement. There is a mass effect with compression of the adjacent corpus callosum and right lateral ventricle

Irregular thickening of the left frontal meninges, up to 12 mm, showing marked gadolinium enhancement.

Signal abnormality in the left frontal bone, appearing hypointense on T1W and T2W, with marked gadolinium enhancement.

Fluid collection in the left frontal sinus.

Case Discussion

The lesions in the brain parenchyma, calvarium and meninges in the left frontal region are consistent with intracranial metastases.
