Presentation

Presented with pruritus, jaundice and cholestasis in laboratory tests.

MRCP images show marked dilatation of the right-left hepatic duct and intrahepatic bile ducts. There is a small obstructive tumor within the bifurcation of the common hepatic duct (open arrow). The common bile duct is not dilated.

Case Discussion

Hilar cholangiocarcinoma (Klatskin tumor), is the commonest type of cholangiocarcinoma. It is a rare malignant tumor arising from the epithelium of the bile ducts.

It has three morphological types: periductal infiltrative, polypoid, and exophytic (mass forming), depending on the predominant pattern of spread in relation to the duct wall.  

Perihilar cholangiocarcinomas tend to present with dilated segmental bile ducts and lack of communication between the right and left hepatic ducts. Other clues to the presence of cholangiocarcinoma include crowding of bile ducts, ductal wall thickening, and lobar atrophy.
