Ground-glass haziness, reticular opacities with traction bronchiectasis (and bronchiolectasis) are noted in both lungs, predominantly in the basal segments. Some degree of volume loss of the lower lobes is noted. Multiple bullae, generally very small, are noted in the right upper lobe. No convincing honeycombing is seen.
Case Discussion
The features are compatible with the fibrotic type of non-specific interstitial pneumonia (NSIP) pattern. The differential diagnosis for this case is UIP pattern. However, the presence of ground-glass haziness and lack of honeycombing make this diagnosis less likely.
