Sessile osteochondroma of the volar surface of the distal ulna. No other osteochondroma is seen. 

Negative ulnar variance. Outward bowing of the radial shaft. Increased radial inclination (ulnar tilt) of the distal radial articular surface giving a pseudo-Madelung deformity.

This patient had a history of hereditary multiple exostoses, which is one of the causes of a pseudo-Madelung deformity
