Radial artery pseudoaneurysm following cardiac catheterization

Presentation

The patient presented with distal forearm swelling after percutaneous coronary intervention and was referred by the interventional cardiologist for a suspected hematoma.

By using a B-scan, show a sac-like lesion at the site of the puncture of the radial artery with swirling motion inside, and by using the spectral Doppler Bidirectional "to and fro," flow is present by systole and diastole respectively seen just proximally to the neck of the sac with the Pepsi sign by using color Doppler.

Case Discussion

Finding by duplex ultrasound illustrated yin-yang sign or Pepsi sign inside cystic-like lesion connected to radial artery proximal to the wrist.

This sign denoted pseudoaneurysm formation of the radial artery, In our case, the pseudoaneurysmal sac was enlarged, and the communication with the radial artery did not successfully thrombose after probe compression. Hence, surgical intervention was needed.

The patient underwent surgical repair by vascular surgery and an interventional cardiologist, which confirmed the diagnosis.
