Axial T2 and DWI show restricted diffusion in the left caudate nucleus and left cerebral cortex.
Case Discussion
The lack of white matter involvement goes against ischemic stroke, which was not the clinical presentation either. Encephalitis is another differential but clinically the patient had only confusion of acute onset.
The patient had positive CSF RT-QUIC test, which is 100% specific for Creutzfeldt-Jakob disease, and the patient had also had a high 14-3-3 protein result.
