Presentation
In this patient there is a history of multiple infections of the upper respiratory tract of unknown cause, in the last months patient debuted with shortness of breath with no apparent known cause, which is why complementary studies were requested.
Patient Data

Hyperclarity involving the posterior segment of the upper lobe of the left lung with decreased ipsilateral vascularity.
Case Discussion
Swyer-James-MacLeod syndrome or hyperlucent lung syndrome has some different presentations as unilateral functional hypoplasia of the pulmonary vasculature.
Although there is no definitive cause, Most authors accepted that bronchiolitis obliterans or pneumonitis and their respective agents are the main cause of this syndrome.
In this patient, we found most of the common changes of this syndrome as reduction in vascularity demonstrated on CT and areas of hyperlucency in this case in apical posteroinferior lobe of right lung.
