Presentation

History of essential hypertension and type II diabetes mellitus.

Patient Data

Age: 70 years
Gender: Male

20 mCi Tc-99m pyrophosphate

Nuclear medicine

Diffuse radiotracer uptake is seen throughout the left ventricular myocardium.

Quantitation of cardiac radiotracer uptake using heart to contralateral lung (H/CL) ratio is 1.74.

Case Discussion

This is a case of transthyretin cardiac amyloidosis as seen on a Tc-99m pyrophosphate study, The heart to contralateral lung (H/CL) ratio was 1.74, where a H/CL ratio >1.5 is determined to be "strongly suggestive".
