Perilymphatic lung nodules follow perilymphatic channels and on imaging are typically subpleural, occur along fissures (perifissural nodules), interlobular septa, and adjacent to the bronchovascular bundles.
Differential diagnosis
Lung nodules in a perilymphatic distribution can be seen in association with a number of conditions 1-3:
sarcoidosis (classic association)
lymphangitic carcinomatosis: tends to be interspersed with interlobular septal thickening
lymphoma (perilymphatic distribution is one of the many pulmonary manifestations of lymphoma)
coal worker's pneumoconiosis: usually simple coal worker's pneumoconiosis
