Milch classification of lateral humeral condyle fractures

Last revised by Joshua Yap on 23 Mar 2023

The Milch classification is one of the classification systems that can be used for lateral humeral condyle fractures and splits these fractures into two groups depending on their relationship with the trochlear groove:

  • type I: fracture passes lateral to the trochlear groove into the capitulotrochlear groove, Salter-Harris IV, stable

  • type II: fracture passes through the trochlear groove to apex, pathologically Salter-Harris IV (but radiologically appears as Salter Harris II as cartilage is not visible), unstable

In general, this system is rarely of radiological use but is used by some orthopedic surgeons.
