There is volar rotation and displacement of the lunate, with loss of the radiolunate and lunocapitate articulation. The remaining carpal bones maintain their alignment with the radius on lateral view. The lunate has a triangular appearance on frontal view with the apex overlapping with the capitate. Findings are keeping with lunate dislocation. No fractures identified.

This is an example of a stage 4 carpal dislocation (perilunate instability).
