Presentation
Pain and swelling of the right thigh.
Patient Data

There is a broad based eccentric lytic lesion in the diaphysis of the right midshaft of femur involving the lateral cortex with periosteal reaction. There is also evidence of thickening of the adjacent cortex. Soft tissue opacity mass is seen anteriorly, suggesting an extraosseous soft tissue component.
Case Discussion
Biopsy reveals Intermediate to high grade osteosarcoma with prominent cartilaginous component.
Periosteal osteosarcoma arises from the inner layer of periosteum (parosteal osteosarcoma arises from outer layer) and contain large amount of cartilaginous matrix.
Periosteal osteosarcoma are worse than parosteal osteosarcoma but better than conventional osteosarcoma in terms of prognosis.
