mri

MRI demonstrates a mass centered on the roof of the fourth ventricle. It enhances heterogeneously but vividly and has restricted diffusion on DWI/ADC consistent with high cellularity. 

There is evidence of extensive leptomeningeal spread. 

Case Discussion

This case demonstrates typical appearances of a medulloblastoma arising from the roof of the fourth ventricle with prominent leptomeningeal spread.

Restricted diffusion, extensive supratentorial leptomeningeal spread, apparent origin from the roof of the ventricle and lack of soft extrusion through the foramen of Luschka and Magendie, all suggest this is not an ependymoma
