Presentation
Abdominal pain, acute jaundice and palpable skin purpura.
Patient Data
Diffuse edematous changes of the bowel loops and mesentery with significant ascites.
No pneumoperitoneum.
Multiple sub centimetric prominent mesenteric lymph nodes are seen.
Homogeneous hepatomegaly is noted with periportal halo.
Edematous changes of the gall bladder with marked mucosal enhancement as well as enhancement of common bile duct wall is seen; however, no dilatation of intra and extra hepatic biliary channels is seen.
The spleen, pancreas, kidneys and adrenals are normal.
Case Discussion
Given the patient's age, clinical presentation and imaging features, the case is very likely for gastrointestinal and hepatobiliary involvement of Henoch-Schonlein purpura, which is also known as IgA vasculitis.
