Lumbosacral soft mass at birth.

Significant fatty tissue around left aspect of lumbosacral spine. In the lower lumbar and sacral planes, the tissue penetrates the spinal canal.

Low position of conus terminalis.

Matching defects in the spinal column.

Impression: spinal dysraphism with tethered cord.

Fatty mass tangential to left dorsal aspect of spinal column.

Sacral spine deformed.

High grade dysraphism.

Case Discussion

Born at 40+3 weeks, normal birth weight and Apgar scores.

Large lumbosacral lipoma seen and palpated at birth. At ultrasound and x-ray, spinal dysraphism with tethered cord was seen. Not seen on prenatal ultrasounds.

Features are compatible with lipomyelocele with tethered cord.
